Our Office Policies

We look forward to seeing you on your scheduled appointment date. If you are a new patient, information is available on line or can be sent to you in an effort to streamline the check-in process and answer any questions you may have regarding your visit. We will be contacting you a few days prior to your appointment to confirm the appointment date and time, and pre-register your demographic information.  We'd like to remind you to please have a driver available to take you home after the appointment if needed.

When you check-in for your appointment, we will need to copy your insurance card(s) in order to bill your insurance company. We ask that you also bring a photo ID.

 If you are on medication and do not have a list of them (including the names and dosages of the agents), please bring the bottles in for your first visit.

Billing

Advancing Wellness, LLC accepts cash and personal checks as well as the following credit cards:

  • Visa
  • MasterCard
  • American Express
  • Discover

We also accept these payment options :

All co-payments and deductibles are due at the time of service. This includes payment for any surgical procedures, which is usually taken care of in advance. We accept cash, check, and Visa, MasterCard,  American Express and Discover credit cards.

Note for first time, new patient appointments, if payment is necessary for a copayment or service rendered, please be prepared to pay by cash or credit cards. Personal checks are not accepted at a first visit.

 

Appointments & Cancellations

In the event that you need to cancel your appointment, kindly do so within 24 hours of the planned visit. Your consideration in this matter will allow us to accommodate other patients who could benefit from an appointment.

If you do not call to cancel an appointment, we reserve the right to bill a "no show" fee of $25.00.

 

Emergencies

 

Urgent after-hours coverage is provided. The On-Call doctor can be reached by dialing the main number: 732-658-1375.  If we are no longer in the office, there will be a phone number provided in the voicemail message that will allow you to reach the covering physician. 

For medical emergencies, please call 911.
